Public health problems at the community level remain a major challenge in efforts to improve population health status, particularly in rural areas. This community service aims to analyze the health conditions of the community in RW 07 Pasirjambu Village, Pasirjambu Subdistrict, Bandung Regency, and to identify priority health problems as well as formulate community empowerment–based intervention programs. This study employed a descriptive design using a household survey approach involving 71 households selected through accidental sampling. Data were collected using structured questionnaires, environmental observations, and interviews, and analyzed using univariate statistical methods. The results indicated that the main public health problems in the study area included a high prevalence of hypertension, suboptimal household waste management, and low levels of physical activity. Priority setting using the Urgency, Seriousness, and Growth (USG) method identified hypertension as the primary health problem due to its significant impact on non-communicable disease risk and overall quality of life. Contributing factors included limited health knowledge, unhealthy dietary patterns, insufficient physical activity, and inadequate adoption of healthy lifestyle behaviors. To address these issues, integrated promotive and preventive intervention programs were designed and implemented, including free health screening for early detection of non-communicable diseases and the SEHATI program (Synergy of Herbal Education, Plants, and Environment) through the utilization of family medicinal plants. In conclusion, a community-based and integrated intervention approach is highly relevant and effective in supporting health promotion and disease prevention at the community level.
